If you are in the US then no, you will not be charged anything extra. You will pay for the item and shipping from Italy.

Make sure that if you pay for the better shipping that your item actually gets sent by that method. And make sure they pack it securely.

If you live in any other country you will pay what I said above along with either one or more (usually just one though) of the following: taxes, tariffs, or import duties.

What exactly are you trying to buy? PM me a link or description and I'll let you know if it is something I can get in the states for you.
